Registration Procedure

1. Complete the Registration Form. (Make sure to check off chosen program)
2. CLICK NEXT AND PRINT THE COMPLETED FORM THAT WILL FOLLOW. (The form will be sent to us automatically)
3. Fill in S.I.N. in proper field, sign and date the form.
4. Attach the following:
   • Certified copy of your Birth Certificate
   • Achievement Record from the MEES (Ministry of Education)
   • Proof of Quebec Residency (if not born in Quebec)
